Hospital Discharge Team

Our Hospital Discharge team are committed to improving the lives of Milton Keynes residents by ensuring a safe and supportive discharge from hospital to their own homes or in an inpatient setting.

 

As a result, we’re going through an exciting transformation of the discharge pathways in collaboration with our partner organisations so that we can improve the discharge process and the lives of service users through the discharge journey.

 

We’re now seeking talented and experienced social workers to join us on this journey and help improve our admission avoidance and discharge from hospital so that we can improve the lives of people who use our service. We have teams based in the hospital and in the community and you’ll join one of these.

Hospital team

​

The hospital team are based in Milton Keynes University Hospital and is where we assess service users to facilitate discharge from hospital through the different pathways to either their own home or inpatient setting.

 

Community team

​

Is based in the community and is responsible for carrying out assessments to support people being discharged with short terms services to either being independent to having formal support.

 

Our Continuing Health Care (CHC) team work within this team and they complete Decision Support Tools to ensure people eligible for health funding receive it.

 

The Integrated Community Support (ICST) are also part of this team and they work within GP surgeries to put in place preventative and community based services to minimise the need for health and social care and maximise their independence. We pride ourselves in working in partnership with other professionals across health and social care.

The hospital discharge team is a friendly and supportive team made up of knowledgeable social workers, social worker assistants and a support worker. You’ll be joining an organisation that prioritises colleague wellbeing, support and personal development. You’ll have close contact with our management team and we’ll help you to develop your skills and knowledge with extensive training opportunities and through regular feedback.
